Permission to administer prescription medication

For students requiring administration of prescription medications such as ritalin, antibiotics and ventolin for diagnosed asthma (for asthma please provide the school with a copy your child/wards asthma plan).

All medication is kept in the front office to dispense as needed. 

Please be sure to provide all medication in the original packaging.
