Permission to administer non-prescription medication

For students requiring administration of non-prescription medications such as panadol, antihistamines and ventolin for seasonal asthma (for asthma with a prescription please fill out the prescribed medication form).

All medication is kept in the front office to dispense as needed. 

Please be sure to provide all medication in the original packaging.
